Our Services
Skilled Nursing Care
Specially trained registered nurses and licensed practical nurses provide skilled nursing services that commence with an assessment to gather information and develop a care plan. The services are prescribed by a doctor on an intermittent basis and include such things as dressing changes, teaching and administration of medications, intravenous therapy, catheter care, diabetic care, wound care, and teaching the family how to care for the patient in the home.
Rehabilitation Services
Rehabilitation services include physical therapy and occupational therapy. They are provided on an intermittent basis in the home as needed for patients who have functional limitations or sensory deficits.
Home Health Aide Services
Home health aides, or Personal Care Assistants (PCAs) work under the supervision of a registered nurse to provide assistance with non-medical assistance, such as bathing, showers, basic hygiene, oral care, foot care, skin care, and changing linens, as well as general housekeeping duties and simple errands. PCA's can also help with dressing patients and provide assistance with ambulation.
Social Services
The social services staff provides social casework services to patients as needed to augment other services of the agency. Services include the evaluation of social functioning, particularly as it relates to medical problems, and initiation and follow-up of referrals to other community resources as applicable.
